Shanghai Travel Delay Form

Complete this form if you are currently traveling to a program at our Shanghai Center and are delayed in transit.

Do not complete this form if you are arriving early (before the official start date) to your program.

Only complete this form if you have received updated or new flight information to share with the staff. If you are still waiting for that information, hold off until you have it and then send us your new flight details.

Your information will be emailed immediately to the staff in Shanghai so they are aware of your updated flight and arrival information.

Refer to the Print Me document, sent to you by email prior to your departure, for late arrival instructions and local contact information.

If you will not know your final arrival time until after you make a connection, complete this form again or call the Center emergency number on your Print Me document to alert the staff of your final arrival time.
